OCLC Web Site a Rich Resource
for a Variety of Online Forms

Do you need a new authorization number to log in to WorldCat? Are you interested in
ordering a set of cataloging records for netLibrary eBooks from OCLC? Do you want to
apply for cataloging Enhance status?
OCLC has the forms required to meet all of the above needs (and many more) available
online at www.oclc.org/support/forms. At this site, you will find a list of the available
forms. Each title is a link to the electronic form and/or its PDF version.
Some of these forms, such as the Electronic Duplicate Report, are completely electronic;
you fill in the necessary information, submit the form online and immediately receive a
confirmation screen thanking you. Forms requiring network authorization, such as the
Access & Authorization Request, come immediately to BCR where they are then
approved and forwarded electronically to OCLC. This saves a great deal of time over
mailing the form or even faxing it.
Some forms are available only as PDF documents, which require Adobe Acrobat to view
and print from your site. The OCLC Accessions List Order Form, OCLC PromptCat
Order and WorldCat Collection Sets order forms (through which you can order
cataloging records for netLibrary eBooks) are among these. Forms that require
supporting documentation, such as the Enhance Authorization Application, also fall in
this category. These are meant to be completed offline and either mailed or faxed.
In addition to ordering forms at www.oclc.org/support/forms, you will find links to forms that
let you communicate with OCLC. The Support Form allows you to send comments and relay
problems about OCLC products and services directly to OCLC, as an alternative to calling
OCLC's User Network and Support helpline at (800) 848-5800 or BCR at (800) 397-1552. A
link to the OCLC Participating Institutions List is found here, as well as a link to the Internet
Request List, which allows you to subscribe to any OCLC-sponsored electronic distribution
or discussion list.
